There's a bug in the generation of table of contents. The first entry in a chapter is correct, but subsequent entries are listed as the page for the previous entry. For example, if the ToC should be:

Chapter....7
Entry 1.....8
Entry 2.....10
Entry 3.....12

It will actually generate:

Chapter....7
Entry 1.....7
Entry 2.....8
Entry 3....10

Note: this seems to be present in all v5. I had to go back to v4 (I think the one I used was 4.1.6.21) to get a correctly generated ToC.

Bug still present. Should be a simple fix: it looks like page number is inserted *before* setting the current page.
